Submission Instructions
Step 1
Download the prescription form in your preferred format — PDF for printing or the editable Rx form to fill in digitally.
Step 2
Complete all required fields: patient details, appliance specifications, and any special instructions for our technicians.
Step 3
Securely package your impressions or stone models together with the completed prescription form.
Step 4
Ship your case via insured courier to our North York laboratory, or contact us to arrange a scheduled pickup.
